A toxic nodular goitre produces excess thyroid hormone independent of TSH control. Here is how it is diagnosed and managed.
What is it?
One or more autonomous nodules within the thyroid produce hormone independently — causing hyperthyroidism, often in older patients.
Diagnosis
Suppressed TSH, raised T3/T4, ultrasound and thyroid uptake scan showing hot nodule(s).
Treatment options
Anti-thyroid medication to control symptoms, then definitive therapy: radioactive iodine or surgery.
When surgery is preferred
Large goitre with compressive symptoms, cosmetic concern, suspicious features or patient preference for rapid definitive cure.
Outcome
Surgery offers definitive cure. Lifelong levothyroxine after total thyroidectomy.
