Role of parathyroid glands
The four parathyroids control blood calcium. They are usually preserved during thyroidectomy but can be bruised.
Temporary hypocalcaemia
Common after total thyroidectomy — most cases recover within days to weeks. Symptoms include tingling around the lips and fingers.
Management
Oral calcium and active vitamin D supplements as needed, titrated by blood tests.
Permanent hypoparathyroidism
Occurs in less than 1% of cases with experienced surgeons; managed with lifelong calcitriol and calcium.
Monitoring
Calcium and PTH levels are checked the day after surgery and at follow-up.
