Calcium & Parathyroid After Thyroidectomy

Why calcium can dip after thyroid surgery and how it is managed.

Calcium levels can drop temporarily after thyroid surgery because of the four small parathyroid glands embedded in the thyroid. Here is what to expect.

Role of parathyroid glands

The four parathyroids control blood calcium. They are usually preserved during thyroidectomy but can be bruised.

Temporary hypocalcaemia

Common after total thyroidectomy — most cases recover within days to weeks. Symptoms include tingling around the lips and fingers.

Management

Oral calcium and active vitamin D supplements as needed, titrated by blood tests.

Permanent hypoparathyroidism

Occurs in less than 1% of cases with experienced surgeons; managed with lifelong calcitriol and calcium.

Monitoring

Calcium and PTH levels are checked the day after surgery and at follow-up.
