Most Hashimoto's patients are managed medically — but surgery has a role in selected cases.
What is Hashimoto's?
An autoimmune disease where the immune system attacks the thyroid, causing hypothyroidism. It is the most common cause of underactive thyroid worldwide.
Standard treatment
Levothyroxine replacement at a dose titrated against TSH levels.
When surgery is considered
Suspicious nodules within a Hashimoto's gland; large goitre with compressive symptoms; cosmetic concern; persistent symptoms despite optimal medical therapy in selected cases.
Lymphoma risk
Thyroid lymphoma is rare but more likely in long-standing Hashimoto's — sudden rapid growth warrants urgent evaluation.
Outcomes
Surgery for selected indications is safe and well-tolerated. Levothyroxine is continued lifelong.
