After total thyroidectomy, levothyroxine is taken for life. This guide explains how to take it correctly and what to expect.
Why it's needed
The body no longer produces thyroid hormone — levothyroxine fully replaces what the thyroid would have made.
How to take it
First thing in the morning on an empty stomach. Wait 30–60 minutes before food, coffee or other medications. Take it consistently every day.
Dose adjustment
Initial dose is usually 1.6 micrograms per kilogram of body weight. TSH is checked at 6–8 weeks and dose adjusted to keep it in the recommended range.
Interactions
Calcium, iron, soy and certain antacids reduce absorption — separate by at least 4 hours.
Long-term outlook
On the right dose, patients feel completely normal and life expectancy is unchanged.
